The last date for submission of application forms is Saturday 21st February 2015.
• Application forms can be downloaded from AL-Shifa Trust Eye Hospital's. website: www.alshifaeye.org
• Application forms should be submitted along with Bank Draft/Pay Order of Rs. 2,000/- (non-refundable) in favour of "Al-Shifa Trust Eye Hospital, Rawalpindi".
• Two complete sets of all documents (Degrees, Domicile, ID card, 3 passport size Photos) should also be submitted with application form through TCS/Courier Services.
• Incomplete applications and those received after the due date will not be considered. Only shortlisted candidates will be called for interviews. No TA/DA is admissible for appearing in interview.
• Admission test of candidates appearing for interview will also be conducted.
• Those interested in evening program may also apply.
